To that end we have developed a Client Information Questionnaire to assist us in preparing for you. It is designed to give us an idea of your expectations and abilities, enabling us to design a trip specific to your desires. To access your Client Information Questionnaire, complete the form below and click on the 'Get Questionnaire' button. Your individual Username and Pass Code was provided on the final page when you completed our Inquiry/Booking form. If you did not retain a copy of the completed Inquiry/Booking form, contact Tom at tom@bowriverhookers.com to obtain your Username and Pass Code. Please note: both the Username and Pass Code are case sensitive.
